Elite Artists Network supplies the following documents for accounting and compliance purposes:
Completed W-9 Form
A current W-9 is available for all contracted performances and can be submitted directly to your accounting department upon request.
Custom Certificate of Insurance (COI)
We offer tailored Certificates of Insurance that meet your venue’s specific requirements. COIs can include additional insured wording, specific coverage limits, or venue-specific formatting as requested.
To help streamline your internal approval and scheduling process, we can also furnish:
Performance descriptions and artist bios
Show schedules and set lengths
Stage plots and technical needs
Arrival and setup timelines
Contact information for onsite communication
If your venue has its own forms or documentation requirements, simply send them to us and we will complete and return them promptly.

Technical contact or AV manager details
Venue load-in instructions or parking details
Stage dimensions and floor plan (if available)
Preferred arrival windows or noise restrictions
